Reporting Ordination and Licensure

Policy Section 1140

The officiating minister is responsible to see that a report of the ordination or licensure is promptly sent to the CMC office, preferably by use of the form"Report of Ministerial Ordination or Licensure in section 3020. The report includes information such as name, address, phone number, spouse, and birth date of ordinand or licensee; ministerial office and status (license or ordination); indication of use of the CMC approved questionnaire; place and date of ceremony; place and assignment/function of ministry; officiating and assisting ministers.

In the case of extension of license, the officiating minister submits a copy of the completed "Ministerial License Extension" form to the CMC office.

After receiving the report from the officiating minister, the CMC general secretary prepares a report which is processed by the Executive Board and is presented at the CMC business meeting.
