Request for Authorization for Ministerial Ordination or License

The polity of Conservative Mennonite Conference requires advance authorization of ordinations and licensures. Requests for authorization for ordination and licensures are processed by the Executive Board and reported to the CMC business meeting. The CMC Ministerial Committee also processes requests for authorization, with recommendation to the Executive Board after evaluation of the candidate's Ministerial Questionnaire.
Requests for authorization should be submitted to the CMC office at least six weeks in advance of the Executive Board meeting or the CMC business meeting in which action is desired. The candidate's Ministerial Questionnaire should accompany this request form.

Each signer should sign his name, indicate office of responsibility in the congregation or agency, and date of signature. The signatures of the overseer, senior pastor, and associate pastors should be included in the case of congregations where such are in place, along with the signature of officiating minister. Signatures of the congregation's board of elders or the equivalent thereof are not required, but may be included.
